^Cholesterol on medication

Annual - Blood test

Diabetes

Annual - BP, weight, blood test, Urine- ACR, hospital eye check for retinal screen, foot check by the nurse. Complete online medication review.

(Uncontrolled diabetics may require more close monitoring of blood test at least 6monthly)

Epilepsy

Annual - Bp, weight, blood test, Complete online medication review

Heart conditions: High blood pressure, stroke or other heart conditions (e.g. heart failure)

Annual - BP, weight, blood test, Complete online medication review.

Heart conditions: Atrial Fibrillation

6monthly - blood test if on DOAC blood thinner, more regularly if on warfarin.

HRT (hormone replacement therapy)

Remain breast aware, unusual periods

Kidney disease

Annual or 6monthly depending on progression - BP, weight, blood test, Urine- ACR

Learning disability

Annual - BP, weight, blood test, annual nurse review

Mental Health requiring medication

Annual - BP, height and weight, blood test, specific drug level monitoring (depending on medication taken), (ECG in some cases). Smoking status.

Oral contraception- the Pill

Annual - BP, weight, Complete online medication review

Remain breast aware.

Respiratory conditions; asthma and COPD

Annual review for inhaler check and check control of condition with score (asthma control test ACT score, COPD assessment test CAT score). Smoking status. Complete online medication review

Rheumatoid Arthritis

Annual - BP, weight, blood test, specific drug level blood test (depending on medication taken). Some medication may require regular eye test. Complete online medication review
