If you have been seen by another institution who have provided a letter (eg optician- GOS18 form) to forward to practice about an onward referral, please complete eConsult and take a photo of the document to add as an attachment. We may need further clarification before we can act on any requests.

If you have been seen by a NHS specialist at the hospital. They can arrange direct referrals to another department (usually within the same hospital) without involving the practice.

We regret we only accept letters from certain institutions. We can not accept letters from an osteopath/ chiropractor in relation to organising scans- for this you would need to book appointment with our First contact practitioners for review.
