Have you been discharged from hospital and need a care package/ extra support?

If you have just been discharged from hospital and remain unwell, this could be considered a failed discharge. If you remain very unwell you will need to reattend the hospital you were discharged from.

When you are discharge from hospital, a social care pack is arranged by the hospital which may include social care arrangements or district nurse. You can find the the district nurse team details here.

Any medication changes should be informed to both your pharmacist and the surgery so that your repeat prescription list remains current.

If you have had major surgery or are recovering from a major health event, please contact the surgery so that we can check on your recovery. You can do this by completing an online consultation form. We may need to update your records and keep monitoring on how you are doing via our care coordinator.
