Registration times

Registration available between 10.00am and 4.00pm (contact reception for further details)


Identification

To complete your registration please bring/ send us the following documents:

  • Proof of Address (utility bill/bank statement/ tenancy agreement) dated within the last 3 months

  • Photo ID (Passport/Driving License)


Your regular medications

  • Please make sure you have your repeat prescription reorder form (from your previous GP). We need to see all medications with your name on it; needed to verify what they are and confirm the dosage.

  • All new patients who are taking regular medication are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.


How soon can I book an appointment

  • You can book an appointment to speak with a doctor as soon as your registration is confirmed by us.

  • Please be aware that it can take time for your previous GP records to transfer over; 2-6wks. This is because the record has to go to PCSE first and then come to the new practice. Despite your record being an electronic record this transfer sometimes fails (this is beyond our control). We would then have to contact your previous practice.


Changes to personal contact details

The best way to update your contact details either by email or via online consultation or via reception.

Mobile or email: Please make sure your email/ telephone contact number is up to date as we may need to reach you urgently for any investigation results. If your have recently changed your personal mobile number or email addresses please update us as soon as possible.

Address: If you change your address please notify the surgery as soon as possible. Failure to do so could result in your removal from the practice list due to mail being returned back to the surgery as undelivered. Additional verification may be required.


Extended leave and registration

If you are going to be outside the UK for any extended period of more than a 3 months duration please inform the practice otherwise on your return you may find that you are no longer registered and will require re-registration. This is an automatic process where status check verification letters are sent to your home address.
